To join the Virtual Patient Group or to volunteer a little of your free time you can: -
Access the John Kelso Web Site www.johnkelso.co.uk and complete the Application Form which can be found on the web site. This form can then be completed and sent to the practice electronically or you can print off the form and place it in the box provided at Reception.
Alternatively
These same forms can be collected from the Receptionist completed and placed in the box provided.
We will then contact you and discuss with you the ways you can help

Who are we and what do we do-
We are a group of volunteer patients involved in working with the surgery to ensure you are provided with the service you need. To date we have been involved in the following:-
The Notice Boards in reception are maintained by us.
We were instrumental in designing and providing the pocket/purse sized card currently available at reception which gives useful telephone numbers.
The blue information kiosk in reception was set up with our help and input.
We help with the preparation of Surveys seeking your opinions about the service offered.
Your PPG is a member of the Cluster Group which covers Leek and Werrington. We meet regularly to exchange views and to discuss issues affecting the service offered.
This Group have been instrumental in seeking a review of the Podiatry Services offered in this area in an effort to improve services. This review is still underway.

There are many ways you can help us. We are always looking for people with new ideas, but realise not everyone has the time to attend regular meetings. So you have choices.
You can, if you wish, provide us with your email address which will give us a chance to consult you from time to time on proposed changes.
By doing this, you will help your GPs to provide an accessible and responsive service and you will be amongst the first to hear about news and updates.
